Can the health rating change if I don't rest tired players? Ie, a B- Health rating goes down to a C+ permanently because I played this guy when he was tired, or does this rating only affect the likely hood a player will get hurt, regardless of his fatigue?

Health doesn't change. It may effect the liklihood of injury, and the quickness of a player tiring. A C+ health player will get considerable less AB's than a B+ health player meaning less improvements. The injuries alot of times seem to be a crap shoot.
