skip 5th starter option

what ever happened to this option. Real baseball uses it. They have spot starters all the time.

There were problems with that setting so it was simply dropped in favour of the "skip starters" option which accomplishes the goal of always having your best starter pitch when rested but takes some starts away from #4 as well.
