Walking through the code manually I can't see how a timeout wouldn't have been called.

There is code to NOT call a timeout when setting up a field goal so as not to give the opponent more time, but when there are less than 20 seconds on the clock, that is overridden. Furthermore, the team should have been in hurry up mode and in hurry up mode a field goal can be completed in 15 seconds. So there are two different ways this shouldn't have happened.

Looking into it.

I've found and fixed the "hurry up" bug, still looking for the timeout bug.

I've found and fixed the timeout bug... it was very subtle.

As noted, the game doesn't want to call a timeout on a field goal unless it has to. At the time the code was first written, a field goal couldn't be completed in less than 20 seconds, so with less than 20 seconds left it would call a timeout. However, that logic ran at the beginning of a play, so the 5-7 second reset between plays had to tick off before a timeout could be called. So the timeout logic was moved to before that 5-7 second runoff (which is part of the prior play), at which point there was 21 seconds on the clock. It decided a timeout wasn't needed and time ran out because of the hurryup bug.

This is fixed and the fix moved over to the main site. Thank you!
