Heaven forbid you lose your top two qb's during a game, but if so, would the 3rd play out of the inactive roster (like the NFL) or do they have to be active prior to the game starting?

I think they have to be active, but it should be set up the other way, so that your 3rd QB can be listed as "inactive" but can still be eligible to play. But, if the 3rd QB enters the game, I think it disqualifies the others from coming back in.

In the NFL, at least, that only applies if the 3rd QB comes in before the start of the 4th quarter

That's what I was seeing and thinking. I'm going with 2QB's active and see how it goes.

My 3rd QB is my best holder so I have him active. Seems like I can do without someone else. I haven't had injuries or rest come close to decimating me at any position yet.

I have not implemented the "extra QB" rule because it complicates the code quite a bit for an extremely rare situation. If you lose all of your QB's, it is likely that your punter or an RB will come in at QB as those are the positions most likely to have some passing skills.
